My doctor advises me to create a Shared Medical Record (DMP) for my children. What is the point and how should I proceed?
Service-public's response:
DMP makes it easy to share health information that could be useful in caring for your children. You can create it in a pharmacy, with a health professional, at the reception of your Primary Health Insurance Fund or with an advisor of your health insurance organization.
This digital health book keeps the memory of your children's health for better medical follow-up throughout their lives (history of care of the last 24 months, treatments followed, results of examinations, medical history, reports of hospitalization, etc.) and thus promotes coordination of care between all health professionals. You can add useful information such as blood type, contact information for people to contact if there is a problem, and a photograph of the pages
long-term pathologies
,
allergies
and
family history
from the health book.
It will be particularly useful to follow the 20 mandatory health exams that run until the age of 16.
This service is not mandatory, it is free and highly secure.
The DMP can be accessed at any time from dmp.fr (My DMP section) or via the downloadable DMP mobile app for iOS and Android systems.
FYI
In 2020, the vaccination booklet will complement the DMP.
