Care Sheet

What is a care sheet? It is an electronic form (vital card) or a paper sheet. It proves the reality of medical expenses. This form is therefore indispensable to be refunded. Be aware that the refund procedure depends on the nature of the form. We'll lay out the rules for you.

Vital Card

The Vital Card allows the healthcare professional to upload an electronic healthcare sheet to your health insurance organization.

You give the healthcare professional you are consulting your vital card.

The healthcare professional writes on his computer the information (your identifier, date of the act, amount of the fees...) that allows to see the billing of the care and thus to trigger their reimbursement.

Please note

Your minor child may be attached as a person entitled to your Vitale card.

It then transmits this information by computer to your health insurance organization.

So you don't no steps to take to be refunded.

The refund period is normally one week without the need to send a care sheet.

FYI  

In the event of failure to issue the electronic healthcare worksheet, the healthcare professional prepares a duplicate (duplicate) paper of the healthcare worksheet.

You may have an indication of the estimated processing time from your friendly account.

The refund statement is available on your account 24 hours after your refund.

Paper Sheet

The paper care sheet is essential to get a refund if you forgot your Vital card or if the healthcare professional is not equipped for the Vital card.

You're the insured

The correct filling of your care sheet is the guarantee of your reimbursement by the Health Insurance.

If you pay your doctor, you must provide your Social Security number and sign the care slip before sending it to your health insurance organization.

To complete the care sheet, you can see package leaflet on the back and seek advice from the healthcare professional you are consulting.

You're not the insured

If you are the person receiving care but not the insured, you must provide the following information:

  • Your first and last names
  • Your Social Security number (if you have one)
  • Your date of birth
  • Surname, first name, address and social security number of the insured person on whom you depend

You must sign the care sheet before sending it to your health insurance organization.

General case

If your healthcare provider has provided you with a paper care sheet, you have Maximum 2 years to send it to the health insurance and get you reimbursed. This period corresponds to the period of validity of the care sheet.

Disease

If your reimbursements relate to an illness, the time limit starts on the date of care and expires at the end of the same calendar quarter, 2 years later.

Example :

if you see your doctor on july 6, 2024, you can submit your care sheet until september 30, 2026 (end of 3rd trimester 2026).

Maternity

If your refunds are for maternity, the two-year period shall start on the day of the firstre medical assessment of your pregnancy.

If you send the care sheet to your CPAM, it is sent by postage-paid mail.

The CPMA tells you based on your postal code where to send your care sheet.

You can also drop it off directly at a reception point in your department.

FYI  

In case of a lost or unreceived care sheet, you can ask the healthcare professional for a duplicate (duplicate) of the care sheet.

If a prescription has been prescribed, you must attach it to the care sheet to be reimbursed. This is the case when the care or products require a medical prescription.

Please note

Health Insurance partially reimburses your health expenses. So you have to think about settling televising with your mutual.

Reminder

You have Maximum 2 years to send it to your primary health insurance fund (CPAM) and get refunded.

You can get an indication of the estimated processing time of your request from your friendly account:

  • "My Approaches" section
  • Then “Check the processing times of my CPAM” by filling in the date of sending / filing your mail.
