Reattachment of a child as a person entitled to social insurance

A person who derives his or her right to sickness and maternity insurance by virtue of his or her relationship with an insured person
A person designated to exercise a measure of protection of an adult or minor (e.g. to represent him in the day-to-day acts of life, to manage his property)
What applies to you ?
Can a minor be entitled to the status of a person entitled to social insurance with one or both of his parents? Yes, however, at age 16, he can apply to become a self-insured.
Please note
Any person who deliberately refuses to become affiliated or who persists in not committing the procedure for compulsory membership of a social security scheme shall be punishable by six months' imprisonment and a fine of €15,000, or only one of those two penalties.
Before age 16
A child under the age of 16 may be reunited as a of entitled person :
- Or to a parent who is responsible for it
- Or to the 2 parents who are responsible for it (double attachment).
The application for reunification must be made to the health insurance body of the parent(s) concerned on the cerfa form No 14445, when the child arrives at the home.
A copy of the family record (or the child's birth certificate) must be attached to the application for attachment. Possibly a copy of the document certifying that you are guardian of the child or that you have collected it.
Application for the attachment of children to one or both parents - Sickness and maternity insurance
Before age 16, the child's health expenses are reimbursed to you. He's your successor.
In the case of a double connection, reimbursement of the costs shall be paid into the account of the parent who submitted his or her vital card.
If no reattachment request is made, the parent performs the 1re claim for reimbursement of healthcare costs.
If one of the two parents ceases to be covered by social insurance, the child's refunds are paid to the other parent.
Between 16 and 18 years old
A child aged between 16 and 18 years may:
- Or be attached as a of entitled person to one of his parents or to the 2
- Or apply, from the age of 16, for the status of a person entitled to stand alone.
At the age of 16, a child can apply for social insurance, regardless of his or her situation.
In that case, he shall cease to be attached as a person entitled to receive benefits and shall, in his personal capacity, be responsible for his health costs in the event of sickness or maternity.
On the other hand, if the child has not made the application for entitlement to stand-alone entitlement, he retains the status of entitlement of a social insured person.
The child's health costs are reimbursed to the insured person to whom he or she is connected. He is entitled to this insured.
In the case of a double connection, reimbursement of the costs shall be paid into the account of the parent who submitted his or her vital card.

Entitled to one or two parents
In order to be attached as a person entitled to one or both of his parents, the child must not engage in any professional activity.
The application for reunification must be made to the health insurance body of the parent(s) concerned on form cerfa No 14445.
The form allows you to request the attachment of a child:

- Or to the 2 parents (double attachment).

Autonomous entitled
The application for an autonomous successor in title shall be made by simple letter to the CPAM: titleContent to which its parent is affiliated.
If, from the age of 16, the child has applied for entitlement to stand-alone rights:
- He receives his refunds from his own bank account.
- He receives his own refund statement.
- He has his own Ameli account.
A student, aged 16 to 18, in his capacity as an autonomous beneficiary, shall automatically receive the refund by transfer to his bank account.
Please note
A student between the ages of 16 and 18 is automatically identified as a stand-alone student.
FYI
From the age of 16, the primary health insurance fund contacts the person entitled directly and sends him the form "My new vital card".
At 18
Entitlement ceases on the day the child turns 18.
A form must be used to claim health costs (sickness or maternity) on a personal basis.
Application for payment of personal health costs in the event of sickness or maternity
The child must attach to this form his or her bank identity statement, written in his or her first and last name.
The file must be mailed to the CPAM: titleContent.
- The adult receives his or her refunds from his or her own bank account.
- She gets her own refund statement.
- She has her own Ameli account.
