Maternity Health Card (or Pregnancy Card)

You are pregnant and have a maternity health card (also called pregnancy record book) has been given to you? It is intended to provide you with clear information about the course of your pregnancy. It is sent to you free of charge by your department at the end of 1er prenatal examination. Every examination is recorded there by the doctor or midwife. This facilitates communication between health care practitioners. We present you with the information you need to know.
The Maternity Health Notebook provides information on the following:
- The course of the medical follow-up of your pregnancy
- Your rights (example: maternity leave)
- Your obligations (example: medical examinations)
- Existing aid (example: early childhood care benefit).
It makes it easier for health professionals and social workers who follow your pregnancy through to delivery to coordinate the follow-up of your pregnancy.

The Maternity Diary is currently being updated. The updated model will be available online in 2024.
The maternity health booklet is issued to you for free by the services of your department during the 1st prenatal examination mandatory, i.e. before the end of your 3e months of pregnancy.
During each medical examination, the doctor or midwife records her findings and information on your maternity diary.
The notebook is your property : no person, including the future father, can demand its disclosure.
Professionals who consult it and enter annotations on it shall be subject to professional secrecy.
