Application for the 20% Surviving Spouse Annuity Supplement (Form 60-3555)

Allows the person who lived as a couple accident at work with an employee who has died as a result of an accident, accident or occupational disease, to apply for the pension supplement 20%.

2 conditions must be met: you are 55 years of age and at least 50% incapacitated for work.
