Single Application for Basic Personal Retirement - For all activities covered by the Pension Insurance, the MSA and the Religious Scheme (Form)

This form allows you to apply for your basic pension from one organization for all of the following activities that you may have performed as:

• Employee or contract civil servant, head of company, collaborating spouse, craftsman, trader, industrialist, etc. (Pension insurance)
• Agricultural worker, farm manager or farm company, worker and family helper (MSAMSA : Agricultural social mutual society)
• Unregulated liberal pension insurance professional
• Minister of Religious or Religious Affairs (Old Age Invalidity and Sickness Insurance Fund - Cavimac).

Unless otherwise indicated by you, this also applies to your compulsory supplementary pension as:

• Farm or company manager, employee and caregiver
• Head of company, collaborating partner, craftsman, trader, industrialist, etc.

You can submit a single application for personal retirement if you meet the following conditions :

• You have reached the legal retirement age
• And you have at least 1 term of insurance with your Pension Insurance or MSA
• Or you have at least 8 quarters of insurance before January 1, 1998 or at least 1 quarter after January 1, 1998 with Cavimac.

File your unique request for personal retirement at the earliest 5 months before the date you have chosen for your departure.
