Registration in the national organ donation refusal register (Form)

If you do not wish to donate your organs and tissues after your death, register on the National Refusal Register. Registration can be done online or by mail.

Your registration request is:

  • Dated and signed
  • Accompanied by a photocopy of a two-sided document proving your identity (examples: valid national identity card, passport expired less than 5 years ago, driving license or residence permit).

You can also prove your identity using the FranceConnect online service.

A certificate of registration is sent to you unless you have indicated that you do not wish to receive it.

Please note

Entry in the national register of refusals may be made from the age of 13.
