Certificate of Transfer of a Used Vehicle (Form 15776*02)

The form must be completed by the former owner (seller) and the new owner (buyer), and signed by the two parties, even if it is a gift (or transfer free of charge).

If there are several co-owners, or co-heirs each must sign it.

The seller retains copy 1 and returns copy 2 to the new owner with the other elements necessary for the sale.
