Report and investigation form accident or serious incident in a physical activity and sport establishment (EAPS) (Form 15796*02)

To be completed by the operator of the establishment for any accident or serious incident occurring within the establishment and to be sent within 48 hours to the departmental service of the State responsible for sports (SDJES-DASEN) of the place of the accident/incident.

Warning  

the document must be sent to the SDJES (not to the DDCSDDCS : Departmental Directorate for Social Cohesion). A new version of the cerfa form, taking into account this change, will soon be available.
