The form is to be completed by each applicant.

It is recommended that you save the form to the hard drive and complete it in a computerized manner.

The form, once printed, must include the applicant's original handwritten signature.

The form must be accompanied by supporting documents (presented at the end of the document).

It must be deposited in duplicate at the prefecture. The second copy may be photocopied.

The applicant is asked to provide a telephone number and an e-mail address so that the prefecture or high commission can contact him in case of incomplete declaration.
